Open Doors Transportation Fund Application

This application will submit your request for an Open Doors Transportation Fund Bus Allowance. You must also book your seats with the appropriate arts organization. Schedule information for qualifying performances is available below and contact information for each organization will be provided upon completion of this form.

School Information

Select your state below and you will be presented with a list of public school districts and schools to choose from. If your school is not included in this list, tell us about your school in the Comments or Questions section at the bottom of this application so we can process your request.

PLEASE NOTE: This is not your personal employee number. Please contact your school’s bookkeeper for a W9 form containing your school or district’s EIN number. If this will be your first time to attend a student matinee performance at the Kauffman Center, please fax the W9 form to 816-994-9040. We must receive this form in order to process your reimbursement.

The Open Doors Bus Transportation Fund is available for classes of 20 or more students and only covers the cost of student transportation.

The amount of your bus fund allowance will be calculated based on the number and grades of students attending a performance. Your actual bus costs may vary depending on your school’s transportation policies.
Adults are not eligible for the bus fund allowance, even if they are traveling on the bus with the students.

Don’t forget to reserve your seats for your preferred performance directly with the arts organization.

After you submit your application, you will receive an email confirmation. We will review your application and you will receive a separate email within 2-3 business days confirming the amount of your bus transportation allowance.
